In order to receive a mammogram at the CBC, a woman must have an order from her physician. You may bring this in at the time of your visit or your physician can fax it to our center.

If you need financial assistance to cover breast imaging, please contact our outreach and education director at (843) 556-0116 to see if you qualify for our grants that cover the cost of testing.

Be aware there are many different types of insurance. You are responsible for verifying your breast health coverage and any deductibles or co-pays.

Forms

Please print the forms below and fill them out prior to your appointment, or arrive 20 minutes early.

Paperwork is still a normal process in our office. For your first visit you will need to fill out the following forms: Patient Information Sheet, the Charleston Breast Center Notice of Privacy Practice and the Charleston Breast Center History Form.

If you are an established patient you will need to update the patient information sheet for each visit.

If you have had breast imaging done previously at another facility, we will need these films for comparison. If you have these, please bring them to your visit. If you need to request these from a facility you may fill out the Request for Imaging Form and we will send in the request.

For patients needing an MRI, the MRI Contrast Safety Screening Form needs to be filled out. If you are a new patient, you will also need to fill out the first visit forms mentioned above.

If you need to have copies of your breast imaging done at the CBC, please fill out the Release of Medical Information Form so that we fulfill your request.

Outreach / Education Programs

Our objectives are to address service gaps and unmet needs in our community and to reduce breast cancer deaths through education, providing breast screening and diagnostic services and offering outreach support for women.

All women in our community are at risk for developing breast cancer, as most of all newly diagnosed women have no risk factors other than being female. Breast cancer screening using mammography is shown to reduce breast cancer deaths. Ensuring access to high quality mammography services is critical for all women in our community.

Our outreach and education director is responsible for educating the public regarding breast care. She goes into the community and provides education for patients and health care providers. This is one aspect of this program. The second component of the program is patient services.

To this end, several groups of women need specific help in affording this quality-of-life service—breast care. Because of this community need, we have a specific program geared to these patients. Minorities and underserved women represent a significant portion of our outreach program thus far. Furthermore, while we partner with the Best Chance Network (BCN), women do not always meet the BCN criteria. We are experiencing younger women who need lumps evaluated. BCN coverage starts at age 40. These younger women, who do not have insurance or are underinsured, need the peace of mind to seek treatment without worrying if they can afford breast care diagnoses. In these tough economic times, the same holds true for uninsured or underinsured women. Many of these women may likely forgo care due to the cost factor and worrying how to pay for it. Delaying a diagnosis may be devastating. Over half of breast lesions for our patients were found by patients themselves. While not all of these have resulted in a cancer diagnosis, being able to reassure scared patients if something is benign or malignant is a significant value.

The CBC is partnered with other not-for-profit organizations throughout the region so that these services can continuously be available to all women in need. Fortunately, we receive grants and donations from individuals that are used to subsidize breast imaging service costs for women who cannot afford to pay for this and who do not qualify for any subsidy programs.
